Eligibility

You are eligible for benefits when you work at least 30 hours per week. Your benefits are effective on your date of hire. You may also enroll your spouse, same gender or opposite gender domestic partner, and your children or children of your domestic partner up to age 26.

Medical Plans (BCBS Alabama)

You have two medical plan options to choose from – both are on the Blue Cross and Blue Shield network and pay 100% of the cost for preventive care.

  • PPO Plan (the Buy-Up option): $1,000 Singlde Deductible, $40 PCP Copay, $20 Generic Drug Copay.

  • CHDP Plan with Health Savings Account (HSA): $2,800 single deductible (must be satisfied before 20% coinsurance applies). This plan includes a $750 OR $1,500 employer contribution to your HSA. 

Dental Plan (BCBS Alabama)

The dental plan is on the Blue Cross and Blue Shield Access Plus Network. The plan pays 100% of the cost for routine checkups and shares the cost for most other dental procedures.

Up to $500 of your unused annual maximum rolls over into the following year!

Vision Plan (EyeMed)

The vision plan is on the EyeMed network and offers both in-network and out-of-network benefits to help you pay for the cost of eye exams, glasses and contacts. The plan pays for one eye exam, one pair of lenses (or a contact lens allowance) per year, and a new frame allowance every two years.

Health Savings Account (HSA) (HealthEquity)

When you enroll in our Consumer-Driven Health Plan (CDHP) (e.g., High Deductible Health Plan), you will open a Health Savings Account (HSA) through Health Equity.

You can make contributions to your account through pretax payroll contributions. Therapy Brands will also contribute to your HSA on your behalf.

Flexible Spending Account (FSA) (HealthEquity)

Health Care and Dependent Care Flexible Spending Accounts with Health Equity allow us to contribute funds on a pretax basis to help pay for medical, dental, vision or Dependent Care costs when paired with the PPO plan.

Voluntary Life and AD&D Insurance (Unum)

You may also purchase Life and AD&D insurance for yourself, your spouse, and your dependents up to certain plan limits.

  • Yourself: the lesser of 5x your annual salary or $500,000
  • Your spouse/domestic partner: $250,000 (up to 100% of your benefit amount)
  • Your child(ren): $10,000

Short & Long-Term Disability Insurance (Unum)

You may purchase disability income benefits through after-tax payroll deductions.

Short-Term Disability – 60% of pre-disability earnings

  • Up to $1,000 per week

Long-Term Disability – 60% of pre-disability earnings

  • Up to $5,000 per month if you earn <$100,000 annually
  • Up to $10,000 per month if you earn >$100,000 annually

Accident Insurance (Unum)

If you or your covered family members are injured in a covered accident, Accident Insurance pays you a lump sum amount to help take care of unexpected costs.

Critical Illness Insurance (Unum)

If you, your spouse/domestic partner, or child are diagnosed with a covered illness, this benefit will pay a lump sum directly to you. Your child(ren) is covered automatically when you elect coverage for yourself.

Hospital Insurance (Unum)

Hospital Insurance pays a set amount directly to you for the hospital admission, ICU admission, or daily hospital stay of a covered family member.

ID Theft Protection (Allstate)

Identity Theft Protection works to keep you and your family’s personal data protected, secure, and most of all, private.

Pet Insurance (Nationwide)

Covering dogs, cats, birds, reptiles, and other exotic pets. Pet Insurance offers 3 levels of reimbursement for expenses related to your pet’s accidents, illnesses, or wellness.

Legal Insurance (ARAG Legal)

Take control of life’s legal matters with access to a network of trusted attorneys who can advise and even represent you, as well as easy to use online tools if you prefer to do it yourself. Your spouse/domestic partner and children (unmarried, up to age 26) are covered automatically when you elect coverage.

Employee Assistance Program (EAP) (Behavioral Health Systems)

The EAP offers free and confidential counseling and assistance with personal, family, and work-related matters for you and your immediate family. Each member receives up to 5 sessions per year, and most services are available at no cost to you.

401(K) Retirement Savings Plan (Empower)

You are eligible to enroll into the 401(k) Plan as of the first day of the month following completion of ninety days of service with the Company (auto enrollment applies). You can make contributions from your pay on a pre-tax or post-tax (e.g., Roth) basis up to the IRS annual limit. Therapy Brands matches 100% of the first 3% and 50% of the next 2% you contribute on an annual basis.

Broad Based Employee Equity Program

Therapy Brands is offering all employees the ability to participate in the future equity value of the Company. Upon a future liquidity event of the Company, 2% of the equity profits realized in the transaction will be awarded to employees through a profit pool cash distribution. To participate, you must be employed by Therapy Brands or a subsidiary / affiliate company at the time of the closing of the liquidity event. Your manager or People Operations can share with you more detail about your specific role’s eligibility.

Holidays

Full-time and part-time employees are eligible to receive holiday pay for designated Therapy Brands holidays. Therapy Brands recognizes eleven paid holidays each year.

Volunteer Time Off

Therapy Brands recognizes it is our responsibility as a good corporate citizen to help enrich our surrounding communities of residence and work. We encourage you to become active in the community. The Volunteer policy provides 8 hours per quarter of Volunteer Time Off per calendar year for eligible full-time employees as a benefit of employment.

Leaves of Absences

Therapy Brands offers a generous leave package for eligible employees. Our leave package includes all federal and state mandated leaves along with additional leave options that allow our employees to best take care of themselves and their families.

Employee Referral Program

Therapy Brands will give a referral bonus to employees who refer qualified candidates for current opportunities posted in the recruiting gateway pursuant to the guidelines identified in the Program guidelines. We place great importance on referrals because we trust our employees know what it takes to be a successful contributor that positively impacts the bottom line. If the candidate you referred is offered and accepts an opportunity, you will be notified by People Operations of your potential referral bonus.
